Cystoscopy
Cystoscopy (cystouresthroscopy) is a diagnostic procedure of viewing a person's urinary passage and the urinary bladder, collecting urine samples and examining the prostrate gland through an instrument. The cystoscope is as thin as a pencil and has a light at the tip. Many cystoscopes have extra tubes to guide other instruments for procedures to treat urinary problems. Through a cystoscope, the urologist can focus on the inner surfaces of the urinary tract. Some cystoscopes use optical fibers that carry an image from the tip of the instrument to a viewing piece at the other end.
Ridid cystoscope: This is a solid straight telescope that is used along with a high intensity light source and a separate channel to allow other instruments to be attached.
Flexible cystoscope: This is used particularly for diagnosis and follow-up of most bladder tumors. It is a fibre optic instrument that can bend easily and has manoeuvarable tip that makes it easy to pass along the curves of the urethra.
Cystoscopy is also done to evaluate problems that cannot be seen on x ray. Cystoscopy is used to further investigate the problems detected by ultrasound or intravenous pyelography. In women, cystoscopy helps to reveal uterine prolapse. A common and temporary side effect of this test is swelling in the urethra. A urinary catheter may be left in the patient's bladder to help drain the urine until the swelling in the urethra subsides.
Pelvic Fracture
Fractures of the pelvis account only for about 0.3-6% of all fractures. A pelvic fracture can simply be described as a break in one or more bones comprising the pelvis. Pelvic fracture is a serious condition and requires immediate medical intervention.
The worst pelvic fractures are caused by high-speed accidents such as car accidents or motorcycle accidents or falls from high places, which have major impact on the body. The greater the force, the more severe the fracture. Depending upon the direction and degree of the force, these injuries can be life threatening.
Other injuries such as broken bones or damage to liver, kidneys or other organs.
Pelvic fracture also occurs in people with osteoporosis.
Pelvic fracture occurs among teens, involved in sports and athletic activities such as football, hockey, skiing and long distance running. These fractures occur with sudden muscle contractions.
Pelvic fracture is usually caused by falls in elderly people, especially when getting out of a bathtub or descending stairs.
Symptoms of pelvic fracture include severe pain in the groin, hip or lower back area. The pain is bound to worsen when moving the legs. There may be pain in the abdomen and numbness and tingling sensation in the groin or legs. Bleeding from the vagina, urethra or rectum is often noticed with pelvic fractures. There may be difficulty in urinating and difficulty in walking or standing.
Types of Pelvic fractures
Stable or unstable pelvic fractures: In stable pelvic fracture, there is minimal hemorrhage. The break occurs in one point in the pelvic ring. In unstable pelvic fracture, the pelvis becomes unstable. The break occurs in two or more break points in the pelvic ring. There occurs moderate to severe hemorrhage.
Open or closed pelvic fractures: If open skin wound occurs during the fracture in the lower abdomen, it is called open pelvic fracture. If no skin wounds occur, then it is closed pelvic fracture.
Diagnostic tests such as x-rays, CT scans are used to diagnose pelvic fractures. MRI allow a detailed picture of the pelvic area. Abdominal ultrasound is used to find internal bleeding and other injuries within the abdomen. Urethrography may be conducted to check injuries in urethra by means of an injected dye. Arteriography, in which dye is injected in the arteries to check for internal bleeding within the pelvis, is sometimes used.
Treatment to the pelvic fracture depends upon the severity of the injury caused. A pelvic fracture is a serious injury. In some cases, it may be complicated with injuries in other parts of the body and severe shock as well. Sometimes severe internal and external bleeding and damage to the internal organs could occur. In these situations, immediate attempt is made by the emergency doctor to stop internal and external bleeding caused by the injury. In case of minor fracture, the treatment would merely consist of bed rest and painkillers.
Most of the times, surgery is undertaken to repair the pelvic fracture. Healing after surgery can take anywhere between few weeks to several months. Thus a lengthy rehabilitation becomes necessary after an extensive pelvic surgery. Prostate Cancer
Prostate cancer is one of the most common cancers among men. This type of cancer grows very slowly. High levels of animal fat in the diet may have a role to play in the occurence of prostate cancer. It can also have a genetic factor. High levels of testosterone and certain chemical exposures may be responsible for prostate cancer. If detected early, prostate cancer can be successfully treated.
The prostrate gland is part of the male reproductive system and is located underneath the urinary bladder in men. It secretes some of seminal fluid. Male hormones that stimulate the prostate cause the prostate to grow till a boy reaches adulthood. Then it maintains it's normal size. But if the size of the prostate grows beyond it's normal size, it lays pressure on the urethra and affects the flow of urine. Often prostate cancer does not surface as noticeable symptoms or pain. Symptoms of prostate cancer include difficulty in starting urination and need for frequent urination. Men suffering from enlarged prostate may feel pain or burning during urination and ejaculation. Blood may be noticed in the urine or semen. There may be dull pain in the lower pelvic region and intermittent urine flow.
A digital rectal examination can help in detecting prostate cancer. Prostate Specific Antigen (PSA) is tested in a blood sample. This test allows a physician to detect the incidence of prostate cancer at an early staga. Men over 50 years must get a PSA test done yearly. Radiation therapy is used to treat prostate cancer. Radiation destroys cancerous cells and it is used with computer-imaging software so that the exact location of the prostate is attacked. In this way, damage to surrounding healthy tissue can be avoided. Hormone therapy is used along with radiation to combat prostate cancer. Drugs such as leuprolide and goserelin are effective in blocking testicles from receiving messages to produce testosterone. This type of treatment is used to shrink large tumors. Chemotherapy is effective in treating hormone-resistant prostate cancer. Surgical removal of the prostate gland is sometimes resorted to.
